In Baldur's Gate 3 players who reach the Rosymorn Monastery via the Mountain Pass can enter the Githyanki Creche and interact with Ghustil in the infirmary, where the Zaith'isk device is offered as a potential cure for the Illithid parasite.
The encounter lets you admit you are infected or pretend it's someone else; choosing to speak with Ghustil before confronting Vlaakith preserves access to the Zaith'isk and makes it easier to manage Lae'zel's reactions.
Lae'zel can insist on using the device first, and who goes in affects party approval.
Withstanding the Zaith'isk requires very high saving throws (examples cited include 12 Intelligence, 15 Wisdom and 18 Constitution, plus a cited 30 Deception/Persuasion check to exit the device), and success grants a permanent Awakened buff that lets you use Illithid powers as a bonus action.
Failure imposes ability-score penalties that can be reversed by a Mind Flayer specimen.
